The source material for almost all the printed documents quoted in this book can be found following this introduction.
It is strongly recommended to utilise the text of the book in conjunction with this source material.
Under each document is given the number of the reference found in the book. e.g. ref 144 refers to footnote 144 in the book.
For many of the references, a number of sources are given. In general, the order of the source material is in numerical sequence. However, with a few exceptions, only one of these sources is shown.
However, in a few cases there are several items from a particular paper which are scattered throughout the subsequent pages and they have been included in the first reference to that document, and this will result in a slight deviation from this numerical sequence.
Should there be some non-consecutive extracts from a particular paper, they are separated by a horizontal line.
Most of the source material comes from a variety of books. On the first occasion when such material is reproduced, it is accompanied by the cover page of the book. In subsequent cases, just the name of the author is given in brackets after the reference number.
Since the quality of the various documents varies, the quality of the reproduction will vary.
Due to the tight binding of some of the books, the beginning or ending of the lines may be less clear.
Should any of the source material be difficult to read, it should be enlarged on the screen.
